NURSING FACILITY TRANSITION

BWCIL supports Medicaid-eligible nursing home residents who wish to live independently through the Going Home program.
Going Home
This program gives nursing facility residents the opportunity to transition to more independent living arrangements, such as returning to their own home, moving in with family members, finding an apartment, or choosing another option. Nursing home residents may request a transition themselves, or a referral may be made by a family member, friend, or social worker. You can stop the transition process at any time without penalty.
The transition process includes:
• An initial interview and consent form
• Support from the BWCIL Transition Coordinator throughout the process
• Development of a person-centered plan, created by you, the transition coordinator, and the friends
or family members you choose
• Assistance with home adaptations or finding other affordable, accessible housing
• Financial support with one-time expenses such as a security or utility deposit, housekeeping
supplies, and essential furniture
• Assistance in securing supports and services such as personal care, home delivered meals,
transportation, private duty nursing, housekeeping, and support coordination
• Surveys to determine quality of life after transitioning
